Key Role:
Apply project management expertise and business strategies to manage a multidisciplinary team to improve the health status of communities within the state of California, focused on promoting the effective prevention and treatment of chronic disease. Lead a team that works with home health agencies, practitioner-owned or -operated offices and clinics, and other healthcare facilities on initiatives focused on achieving the goals of improving cardiac health and reducing cardiac healthcare disparities, reducing disparities in diabetes care, and improving prevention coordination through use of health information technology and collaboration with regional extension centers (RECs). Establish work plans, manage the recruitment of healthcare providers to participate in quality improvement activities, and manage the recruitment of and coordination with stakeholders and partners to deliver services to providers. Engage in collaborative partnerships with Medicare beneficiaries or their families, patient advocates, or representatives. Oversee the provision of technical assistance and coaching to providers in the aforementioned goal areas. Oversee the establishment and facilitation of collaborative forums for stakeholders, including participating providers, practitioners, beneficiaries or their family members, and patient advocates and partners to share information, resources, and strategies to achieve goals and compliance with reporting requirements.
Basic
Qualifications:
-6 years of experience with healthcare quality improvement, cardiac care, diabetes care, prevention coordination, or use of health IT
-6 years of experience with project management, people management, strategic planning, project planning, coordination of daily activities, data analysis, constructive reporting, and project management techniques and tools
-Experience with change management, including critical-thinking, innovation, and problem-solving recommending successful solutions and facilitating spread and implementation of such solutions for large scale change
-Experience with care delivery in home health and practice-based healthcare settings
-Ability to motivate and lead a task team to provide quality services and deliverables in a timely manner
-MA or MS degree required

-Ability to use effective communication expertise spanning executives to the general public
-Possession of excellent oral and written communication skills
-RN or MD degree preferred
